Full-Time Occupational Therapist / OT
Apex Home Healthcare, part of LHC Group and Optum, is hiring for a Full-Time Occupational Therapist / OT to join our passionate team in Ormond Beach, Florida.
Primary Responsibilities:- Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
- Evaluates the patient's functional status and occupational therapy needs, consults with the physician in the development of the therapy plan of care.
- Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
- Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Required Qualifications:- Current Occupational Therapy licensure in state of practice.
- Current CPR certification.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- Demonstrated ability to manage multiple tasks simultaneously.
- Demonstrated ability to work independently.
- Good communication, writing, and organizational skills.
If the hired individual resides in Florida (office based or telecommuting), this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face-to-face contact with members or employees who will have access to confidential patient data and will require renewal every five years.
Pay is based on several factors including local labor markets, education, work experience, certifications, etc. The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
