• Oversee the development, implementation, and evaluation of individualized patient care plans.

• Coordinate and manage patient care activities, ensuring compliance with clinical policies, procedures, and best practices.

• Supervise, mentor, and support nursing staff and other healthcare professionals delivering home health services.

• Conduct comprehensive and ongoing patient assessments, ensuring timely and accurate documentation in the EHR system.

• Collaborate with physicians, therapists, and other healthcare providers to create an integrated care approach.

• Ensure compliance with state, federal, and accreditation standards governing home health services.

• Provide education and support to patients and their families regarding care plans, disease management, and health maintenance.

• Monitor quality of care, patient satisfaction, and staff performance to identify areas for improvement.

• Support recruitment, training, and evaluation of staff to foster a culture of accountability and excellence.

• Act as a role model and resource for staff, demonstrating clinical expertise and leadership in complex patient care situations.

Candidate requirements

Required:

Current Registered Nurse (RN) license in the state of Florida.

• Minimum 2 years of clinical nursing experience, with at least 1 year in home health or community health.

• Proven home health leadership experience.

• Strong organizational, communication, and interpersonal skills.

• Proficiency in electronic health records (EHR) and computer applications.

Preferred:

Bachelor of Science in Nursing (BSN).

Previous management or supervisory experience in home care (2+ years).
