RN Patient Care Manager - Hybrid Ormand FL

Position

Apex Home Healthcare is hiring for an RN Patient Care Manager in Ormond Beach, Florida.

This hybrid RN Patient Care Manager supervises and coordinates clinical services, provides direct nursing care to patients in their homes as prescribed by the physician, and coordinates an interdisciplinary team. The position is a Mon‑Fri, hybrid schedule with three in‑office days per week.

• Directly and indirectly supervise home health aides and LPNs, provide instruction, and assign tasks.
• Provide clinical services within the scope of practice in accordance with the plan of care and in coordination with other members of the health care team.
• Complete comprehensive assessments (OASIS), medication reconciliation and initial/comprehensive nursing evaluation visits.
• Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
• Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensure patient needs are continually assessed and care rendered is individualized, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
• Review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations as appropriate.

• Current unrestricted RN licensure in state of practice.
• Current CPR certification.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability or financial condition of the client.

• Home care experience.
• Ability to work independently.
• Good communication, writing, and organizational skills.

Salary ranges from $71,640 to $107,460 annually for full‑time employment. In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401(k) contribution (subject to eligibility requirements). Compensation complies with applicable minimum wage laws.
