Apex Home Healthcare is hiring for an RN Patient Care Manager to join our passionate team in Ormond Beach, Florida. This role is a hybrid between office and RN field work, with three in‑office days per week.
Primary Responsibilities- Directly and indirectly supervise home health aides and LPNs, provide instruction, and assign tasks.
- Provide clinical services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with other team members.
- Complete comprehensive assessments (OASIS), medication reconciliation, and initial/comprehensive nursing evaluation visits.
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start‑of‑care visits.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered is individualized, appropriate, reasonable, meets eligibility criteria, and follows physician orders.
- Review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations as appropriate.
- Current unrestricted RN licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Ability to function in any home situation regardless of client demographics.
- Able to work independently.
- Good communication, writing, and organizational skills.
Salary ranges from $71,640 to $107,460 per annum for full‑time employment. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility). All benefits are governed by applicable laws.
