The Psychiatric Registered Nurse delivers safe, effective, compassionately nursing care in accordance with the nursing scope of practice and individualized Plan of Care. This role involves direct clinical care, timely documentation, clear communication, and coordination with clients, families, and clinical leadership to ensure high‑quality service delivery.

Field Case Management

• Assess mental, physical, and emotional status, creating plans of treatment and coordinating care, collaborating with the Nurse Supervisor or Manager of Clinical Practice.

Psychiatric Care

• Conduct evaluations, medication management, and provide therapy in the home setting.

Documentation and Communication

• Report changes in client medical status and emergencies promptly to the Nurse Supervisor or Clinical Manager.
• Complete required clinical documentation accurately and on time using the electronic medical record system.
• Communicate schedule changes such as tardiness, vacation, requested time off, change of status, or reduction in hours to Care Coordinators promptly.
• Confirm weekly schedules with the Care Coordinator to ensure required staffing levels are maintained.
• Perform additional duties as assigned.

• Current, active RN license in the state of practice, in good standing.
• Master’s Degree: RN with Master’s degree in psychiatric or mental health nursing.
• Bachelor’s Degree: RN with 1 year of recent (within 3 years) acute psychiatric nursing experience.
• Diploma/Associate Degree: RN with 2 years of acute psychiatric nursing experience.
• Proficiency with OASIS documentation.
• Strong communication and documentation skills.
• Proven ability to work collaboratively as part of a healthcare team.
• Commitment to patient‑centered, safe, and compassionate care.

• Patient care is delivered in diverse settings, including private homes, assisted living facilities, and clinical environments.
• Regular activities include standing, walking, bending, reaching, and assisting with patient transfers using proper body mechanics.
• Daily travel to patient locations is required.
• Routine exposure to blood, body fluids, and other potentially infectious materials. Universal precautions and PPE are required.

• Assist with patient transfers and ambulation.

Category 1, Regular exposure to blood, body fluids, and communicable diseases. Universal precautions and PPE are required.
