Position SummaryThe Licensed Vocational Nurse (LVN) – Outreach provides direct patient care and care coordination services to patients in community and home-based settings. This position supports outreach initiatives by assisting with case management activities, patient education, chronic disease support, and continuity of care for high-risk and underserved populations. The LVN works under the supervision of a Registered Nurse and/or provider and collaborates with interdisciplinary teams to improve patient outcomes and access to care.
EssentialFunctions
- Conducts patient outreach through phone calls, home visits, and community-based interactions to support engagement in care.
- Interviews patients to obtain health history, assess needs, and identify barriers to care.
- Performs basic nursing assessments, including vital signs, weight, and other screening measures within LVN scope.
- Provides patient education on disease management, preventive care, medications, and treatment plans as directed by providers or RN.
- Assists in care coordination for high-risk patients, including referrals, follow-ups, and tracking patient progress through programs.
- Documents all patient interactions, assessments, and interventions accurately in the electronic health record (EHR).
- Administers medications and immunizations within LVN scope and per organizational protocols.
- Collects laboratory specimens (e.g., blood, urine) and performs CLIA-waived testing as appropriate.
- Assists providers and care teams with follow-up on abnormal test results and care plans.
- Participates in outreach clinics, community events, and health education initiatives (e.g., chronic disease programs, vaccine clinics).
- Conducts home visits to gather clinical and social information to support care planning.
- Identifies and reports concerns such as abuse, neglect, or safety risks per policy and regulatory requirements.
- Maintains infection control standards and follows OSHA and clinic safety guidelines in all settings.
- Assists with maintaining supplies and equipment for outreach activities.
- Supports communication between patients, providers, and departments to ensure continuity of care.
