• 3-5 years of relevant experience
• Capability to pass a caregiver background check
• Valid driver s license, auto insurance, and a clean driving record are required.
• Hours:
  
  $20/hour
• Work Schedule:
  
  M-T-W-Th-F 4pm-10pm (every other Thursday) and 8am-4pm on weekends to meet 40hrs per week.

• Assist with daily living activities, including personal hygiene and independent life skills.
• Prepare delicious meals that cater to her dietary requirements.
• Act as her coach and cheerleader, helping her set and achieve goals while promoting independence.
• Engage in enjoyable recreational activities and outings to keep her connected and active.
• Maintain a cozy and clean home environment through light housekeeping duties.
• Ensure safety and well-being by adhering to policies and documenting her care.

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children s Long-Term Support (CLTS) program. Self-direction allows participants to make their own choices selecting who provides their care and how those services are delivered. While Community Horizons offers guidance and mentorship, we are not the employer of record.

Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork. Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion.
