Occupational Therapist (OTR) - Part Time

As a non-profit Life Plan Community led by Christian values we have served older adults ages 55 and better in the Oshkosh community since 1967. We are dedicated to providing the best resident centered care.

Job Summary:
We are seeking a skilled and motivated Occupational Therapist (OT) to join our dynamic rehab and home health team. In this role, you will assess, plan, and implement individualized therapy programs for both short-term rehab patients and home health clients. The ideal candidate is a caring professional who enjoys working in a team environment and is committed to helping individuals regain or maintain their independence in daily living.

Key Responsibilities:

  • Evaluate patients’ functional abilities and develop individualized treatment plans
  • Provide hands‑on therapy to improve residents’ and patients’ ability to perform daily activities (ADLs)
  • Document progress and adjust treatment plans as needed
  • Educate patients, residents, and caregivers on exercises and strategies to improve function
  • Collaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workers
  • Ensure compliance with regulatory requirements and documentation standards
  • Maintain accurate records using electronic medical record (EMR) systems

Qualifications:

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited program
  • Current state licensure as an Occupational Therapist (OTR/L)
  • Excellent communication, organization, and time management skills
  • Compassionate, patient-centered mindset with a passion for helping others
