Join Our Caring Team as a Licensed Practical/ Registered Nurse!
Are you a compassionate, dedicated Nurse looking to make a meaningful difference in the lives of residents? We invite you to join our team at Evergreen, where we prioritize exceptional resident care in a warm and supportive environment!
This position is 4 shifts in a two week pay period on the PM shift (2:30pm-10:45am) for a total of 15.5 hours per week. This position includes an every other weekend and holiday rotation.

What You’ll Do:
- Deliver compassionate, patient‑centered care to our residents
- Assist with daily living activities, promoting independence and dignity
- Monitor vital signs and health status, ensuring the best possible outcomes
- Collaborate with a dedicated team of healthcare professionals to create individualized care plans
- Educate residents and their families about health management and wellness
What We’re Looking For:
- A current LPN/RN license in Wisconsin
- A genuine passion for helping others and a strong commitment to resident care
- Excellent communication skills and the ability to work collaboratively
- A positive attitude and a compassionate approach to nursing
