Estimated Gross Weekly Pay: 3520.00

RN - Case Management

• Work Monday through Friday from 8:00 AM to 4:30 PM

• Home hospice position covering territory from Fond du Lac through Oshkosh to Appleton

• Most patients located in the Oshkosh city area

• One weekend rotation per month (manager tries to avoid weekend scheduling but flexibility required)

• One on-call shift per month from 8:00 PM to 8:00 AM

• No holiday coverage required

• Up to 1 week orientation based on comfort level

• 2 years of hospice experience required

• First-time travelers accepted if they have hospice and EPIC Remote Client experience

• Required certifications: BLS

• Valid driver's license, insurance, and personal vehicle required

• EPIC Remote Client experience required

Skills required:

end of life care/planning, wound assessments, care coordination, pain/symptom management, triage

• Work with adults only providing end of life patient care

• Technology equipment includes EPIC Remote Client EMR system and Sapphire IV pumps

• Dress code: any color scrubs or business casual

• May float anywhere within assigned territory

• Time off requests must be approved through manager
