Join Our Caring Team as a Licensed Practical Nurse Are you a compassionate dedicated Licensed Practical Nurse LPN looking to make a meaningful difference in the lives of residents We invite you to join our team at Evergreen where we prioritize exceptional resident care in a warm and supportive environment This position is 8 shifts in a two week pay period on the PM shift 230pm 1045pm for a total of 32 hours per week

What Youll Do- Deliver compassionate patient centered care to our residents
- Assist with daily living activities promoting independence and dignity
- Monitor vital signs and health status ensuring the best possible outcomes
- Collaborate with a dedicated team of healthcare professionals to create individualized care plans
- Educate residents and their families about health management and wellness
- A current LPN license in Wisconsin
- A genuine passion for helping others and a strong commitment to resident care
- Excellent communication skills and the ability to work collaboratively
