The Social Service Coordinator supports residents by coordinating home and community-based services, facilitating social programming for the property, and providing short term intervention and case management support consistent with company policies and Department related requirements.
Key Responsibilities- Resident Services & Case Coordination
- Assess residents who may require assistance with activities of daily living, transition support, or a higher level of care, involving family members and other professionals as appropriate.
- Provide crisis intervention on a short-term basis and deliver case management services for residents receiving support from community agencies.
- Coordinate with hospital staff regarding discharge planning for residents returning to their apartments.
- Advocate for residents and build informal support systems involving family members, other residents, and friends as needed.
- Community Partnerships & Programming
- Make referrals to community service agencies and follow up on services.
- Establish and maintain linkages with community agencies offering services to residents.
- Identify educational and health programs that enhance residents' quality of life.
- Publish and distribute a building newsletter and participate in overall planning for property activities.
- Fundraise to support programs and activities in the building.
- Compliance, Documentation & Communication
- Submit monthly reports in timely manner.
- Maintain a current knowledge base of services and programs affecting seniors, disabled residents, or other populations served.
- Develop and update resource directory.
- Use social services software to maintain resident files in compliance with Department requirements.
- Communicate and coordinate with property management staff regarding resident and program issues.
- Serve as a resource to the Manager related to ongoing policy development and social planning within the building.

- Demonstrate sensitivity and responsiveness to cultural differences within the resident population.
- Always maintain confidentiality.
- Participate in professional organizations and training opportunities to secure required continuing education credits and meet HUD/Department requirements.
- Maintain punctual attendance and meet deadlines and scheduling expectations.
- Required:
High school diploma or GED and at least two to three years of related experience and/or training, or an equivalent combination of education and experience. - Preferred:
Bachelor's degree or Associate degree in Social Work, Counseling, Sociology, or another social science field. - Preferred:
Prior experience as a Social Service Coordinator.
- Preferred: MSW or master’s degree in counseling.
- Preferred:
Community organization training or related credentials. - Must maintain continuing education credits and attend HUD training as required.
- Proficiency with Excel, Word, Outlook, PowerPoint, and social services software.
- Ability to maintain resident files and documentation in compliance with Department requirements.
- Ability to sit regularly; use hands frequently; reach, stoop, kneel, crouch, crawl, stand, and walk as needed.
- Maintain resident confidentiality and report any suspected abuse or neglect while taking proactive steps to protect residents.
- Access to reliable transportation.
- Ability to adapt quickly to changing business needs and work methods with minimal notice.
- Ability to work independently while maintaining strong collaboration with property management and on‑site teams to effectively support resident needs and property operations.

The work environment is typically moderate in noise level. The role involves a combination of desk-based coordination, resident interaction, and movement throughout the property and community as needed.
