Position:
Family Program Case Manager – Support Specialist

Description

Provide prevention and intervention services for parents with cognitive or mental health issues and families in crisis.

Deliver direct service support through a team-based approach for individuals with diverse mental health, developmental, and behavioral needs. Responsibilities include coordinating care, leading therapeutic activities, setting treatment goals, and ensuring comprehensive support across settings. This role fosters a positive environment that encourages growth and empowerment.

Additionally, the program pairs at-risk youth with positive role models to deliver support, guidance, and constructive opportunities for young individuals facing challenges in their home, school, or personal lives.
