Job Summary:
The Wound Care LPN is responsible for all assigned nursing care activities to include but not limited
to wound care for adult patients and other clinical activities associated with providing safe patient
care. The LPN is responsible for following all WCC policies and procedures, care protocols and
Diversified Clinical Services Practice Guidelines, as directed by a Registered Nurse, or Physician.
The LPN will be responsible for assisting with patient and wound care data conferences, and
continuing education is required as determined by the Program Director.
Duties/Responsibilities:
- The LPN must be able to demonstrate the knowledge and skills necessary to provide care to patients served on his/her assigned unit as described in unit based competencies, policies and procedures.
- In collaboration with other staff members, provides bedside patient care, implementing the interventions identified in the plan of care, as directed by RN.
- Ensure appropriate patient care documentation.
- Implement and assure compliance with wound center and hospital policies and procedures.
- Maintain familiarity with regulatory standards.
- Report quality of care issues to staff RN or Program Director.
- Ensure availability of necessary equipment and supplies.
- Communicates with all team members regarding therapeutic interventions.
- Communicates with staff and physicians to ensure quality of care.
- Maintains and demonstrates competency in BLS (Basic Life Support), infections control, safety and all unit required skill review.
- Participates in own employee feedback session identifying individual strengths and areas of growth.
- Identifies limitations in own skill/knowledge and seeks consultation from available resources to improve them.
- Demonstrates commitment to ongoing professional growth and education
- Perform other duties as assigned.
• Ability to cost effectively utilize resources and supplies.
• Ability to work effectively and communicate frequently with other members of the patient care team.
• Ability to communicate effectively, providing appropriate and accurate communication nonverbally, verbally and in writing.
• Excellent customer service attitude with effective listening skills.
• Ability to demonstrate positive, supportive and understanding behavior when in contact with guests (patients, visitors, clients, physicians, employees, volunteers, students, etc.)
Education and Experience:
- Graduate of an approved LPN program.
- Current NYS Licensed Practical Nurse
- Current certification in CPR.
- Unit Specific orientation.
- Requires unit specific orientation.
- Occasionally requires assignments to other units/shifts as hospital needs require.
- Requires frequent walking, standing, bending, lifting, pushing and pulling during the work day.
- Requires assembling, using, and maintaining departmental equipment.
- Exposure to patient blood, urine, and other body fluid samples along with other hazardous materials
- Attendance at scheduled date and location ensures that job requirements are met.
- Actively participates in Quality Assurance, CQI, cross-training, in-service programs; attend all mandatory classes, meetings and committees as required; complete department-specific requirements/competencies; accept assignments to other units/shifts as hospital needs require.
- Complies with DOH requirements for annual health assessments and required immunizations.
Employee salary is based on commensurate experience and other qualifications.
