Join Morningstar Residential Care Center, a leading healthcare organization in Oswego, NY, as we seek a skilled and compassionate Licensed Practical Nurse (LPN) to join our dedicated team. About the Role:
As an LPN, you will be responsible for delivering exceptional patient care to our residents, while working collaboratively with our interdisciplinary team to develop and implement individualized care plans. You will also be expected to maintain accurate and up-to-date patient records, participate in quality improvement initiatives, and provide emotional support and comfort to our residents and their families.
Responsibilities:
Provide direct patient care to our residents, including administering medications, performing vital signs, and providing wound care
Collaborate with the interdisciplinary team to develop and implement individualized care plans that promote the interests and well-being of our residents
Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes
Participate in quality improvement initiatives to ensure that our care is evidence-based and resident-centered
Provide emotional support and comfort to our residents and their families, and engage in activities that promote socialization and community building
Demonstrate a commitment to our organization's values, including delivering excellent customer service, promoting the success of others, leading by example, and committing to quality outcomes

Requirements:
Current LPN licensure in the state of New York
Completion of a Practical Nursing program or equivalent
Minimum of insert minimum years of experience years of experience as an LPN
