Long Term Care LPN
Day shift, 6am-6pm or 10am-10pm, including every other weekend. This position requires 36 hours a week commitment.
We are seeking a dedicated Long Term Care LPN to join our team. The ideal candidate should be willing to float between units as needed, ensuring high-quality care for our patients. Prior experience in long-term care is essential, particularly in managing patients with dementia.
Provide nursing care and support to patients in a long-term care setting.
Monitor patient vitals and administer medications as prescribed.
Assist with daily living activities and engaging patients in care activities.
Collaborate with healthcare teams to develop and implement care plans.
Minimum of 1 year of experience as a Licensed Practical Nurse (LPN).
Must hold a current New York State LPN license.
Basic Life Support (BLS) certification required.
