Parent As Teachers Supervisor

Overview

Primary Accountabilities:

The position is responsible for the day-to-day coordination of program activities and supervision of staff for the Parents as Teachers (PAT) program within the Family Services Department. This position ensures that programs are implemented and operate in accordance with federal, state, and local standards, policies, and guidelines. This position is a working supervisor position with 50% of the time dedicated to providing direct patient services, home visitation services, and coordination of outreach activities such as health education, community participation, and other program activities, and 50% of the time dedicated to supervisory, administrative, and program management responsibilities.

Primary Responsibilities:

• Management of daily work activities and supervision of program staff.
• Responsible for managing schedules caseload assignment by grant source and assigning responsibilities and duties to employees in the department to provide services in an efficient and effective manner.
• Ensures employees receive instruction/training needed to perform work duties competently.
• Monitors’ performance as appropriate, provides mentoring/guidance to assist in employee development.
• Ensures that employees are aware of and adhere to all company and program policies and procedures.
• Conducts performance appraisals, completes performance appraisal documentation and meets with employees to provide feedback.
• Completes observations of home visits and program activities in accordance with PAT program requirements and guidelines.
• Responsible for successful implementation of programs.
• Implements program or clinic activities in accordance with applicable program guidelines/expectations to ensure program objectives are met and are compliant.
• Reviews work for thoroughness and accuracy and provide specific instruction to employees on improvement or corrective actions.
• Investigates and resolves complaints or refers unusual problems to the Director of Programs
• Collects data and provides regular updates and reports on program outcomes.
• Provides direct services and coordination of outreach activities.
• Provides direct services as needed for families participating in programs.
• Maintains records that document interventions provided in accordance with program requirements and guidelines.
• Collaborate with other clinic departments as well as community partners to assure that the needs of patients are being met.
• Ability to demonstrate a working knowledge of all Family Services programs and clinic services in the community and provide job coverage for other departmental staff when needed.
• Travels to support program activities and other work‑related activities, i.e. required program meetings, home visits and/or trainings.
• Other duties as assigned.
• Devotes the time needed during the work week to ensure positive patient outcomes and day‑to‑day objectives are met.
• Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior.
• Must always uphold confidentiality regarding CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
