Licensed Practical Nurse

Responsibilities

• Provide direct in-home nursing care to pediatric and adult patients per physician orders
• Administer medications, treatments, and wound care; monitor vital signs and patient status
• Develop, implement, and update individualized care plans
• Educate patients and families on disease management, medications, and home care techniques
• Document all care accurately in electronic medical records and maintain compliance with policies
• Collaborate with physicians, therapists, and the care team to coordinate services
• Respond to changes in patient condition and escalate concerns promptly
• Ensure infection control and safety standards in the home environment
• Support patient comfort, dignity, and quality of life through compassionate, family-centered care
