What You'll Do

As a Transitions Hospice Chaplain, you'll be the spiritual anchor of the interdisciplinary care team — connecting patients and families to the support they need, in the way that resonates most with them. Your responsibilities include:

  • Assessing the spiritual needs of patients, families, and caregivers with sensitivity and timeliness
  • Providing non-denominational spiritual care directly, or coordinating with preferred spiritual advisors and clergy
  • Developing, documenting, and continuously refining individualized spiritual care plans with the interdisciplinary team
  • Educating and consulting with care team members on spiritual and emotional support best practices
  • Building relationships with local faith communities to expand the network of spiritual support available to patients
  • Conducting funeral, memorial, and bereavement services as requested — including annual services for Transitions families
  • Participating in on-call duties on evenings and weekends as needed
  • Maintaining accurate records in compliance with local, state, and federal requirements
  • Showing up as a positive, motivating presence for colleagues and the broader care team

Requirements What You Bring
  • Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree), or degree in counseling, or equivalent experience.
  • Comfort and experience providing non-denominational care across diverse faith backgrounds
  • A calm, grounded presence in emotionally intense situations
  • Strong interpersonal skills and the ability to collaborate within a clinical interdisciplinary team
  • Valid driver's license and willingness to travel locally for patient visits
