Social Worker, Crisis Counselor

Transitions Care is seeking a compassionate Social Worker / LSW Hospice to support patients and families facing serious or terminal illness. In this role, you'll complete psychosocial assessments, develop person-centered care plans, and provide counseling, crisis intervention, and resource coordination. You'll collaborate closely with our interdisciplinary hospice and palliative care team to honor patient goals, promote comfort and dignity, and guide families through complex decisions and grief.

Responsibilities

• Provide psychosocial assessments and care planning for hospice patients and families
• Offer counseling, crisis intervention, and emotional support through end-of-life
• Coordinate community resources, benefits, and practical support services
• Collaborate with interdisciplinary hospice team on goals of care and discharge planning
• Advocate for patient rights, dignity, and culturally sensitive care
• Educate patients and families on hospice philosophy and available supports
• Facilitate family meetings and support difficult decision-making
• Maintain timely, accurate clinical documentation and care plans
• Participate in team case conferences and quality improvement initiatives
• Support bereavement and grief resources and referrals
