Job Summary

The Certified Nursing Assistant (CNA) serves as a caregiver to the resident during the scheduled work period in long‑term care (LTC). Provides resident‑centered nursing care and daily living assistance to assigned residents under the supervision of a registered nurse (RN). Knowledge of and delivers age‑appropriate care related to the physical and psychological needs of the resident as per the care plan.

The CNA is considered a member of the nursing team and is expected to know and be held accountable for following infection prevention and control policies and personal protective equipment use.

Provides assistance with basic health‑care needs including daily living activities such as bathing, toileting, grooming, dressing/undressing, recording vital signs, and providing psychosocial support and other personal care to assigned residents. Assists residents in transferring, repositioning, and walking using correct transfer techniques and equipment, and also provides range of motion and passive exercises. Completes tasks related to food preparation and dining, creating a positive dining experience for the resident.

Assists residents with meals and snacks, providing water and supplements as appropriate.

The CNA aids providers and nursing staff members with procedures, if needed and documents resident interactions and outcomes related to resident care, behavior, activity, and dietary. Communicates changes in the resident's condition and care concerns or responses to the charge nurse. Collects non‑invasive body fluid specimens, gathers vital signs, and provides housekeeping and laundry services to assist residents in keeping living areas clean and operational.

Provides assistance and documentation with wellness and recreational programs and encourages residents to participate. May also participate in departmental or organizational committees and task forces as requested.

Qualifications

High school graduate or GED preferred, but not required.

Minimum age of 16. Prior clinical or nursing assistant experience in long‑term care preferred.

Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of the start date through enrollment in a training program or the process of obtaining certification through endorsement. CNA training and certification are required and provided by the facility.

Satisfactorily complete yearly, department‑specific competency testing and maintain a record of continuing education credits for each year. A minimum of 12 hours of continuing education are required annually. Maintain all department‑specific required certifications.

When applicable, be certified and registered with the Unlicensed Assistive Person (UAP) Registry for applicable states.

Depending on facility requirements, this role may be classified as a Sanford Category II Driver, requiring a motor vehicle report and proof of a valid driver’s license, and be medically qualified to perform the essential driving functions as per the Sanford Driving Policy and leadership request.
