Professional Licensed Social Worker - Age Wise Advocacy & Consulting

Age Wise Advocacy & Consulting is seeking an experienced Licensed Social Worker to join our elite team. We are a privately owned geriatric care management agency serving the Kansas City Metro area. This position is 25-30 hours per week.

The Care Manager will be responsible for managing and delivering comprehensive advocacy services to clients in their homes, within care facilities, and through virtual platforms. The ideal candidate will possess extensive experience in geriatric care management, strong interpersonal and written communication skills, and a passion for supporting older adults.

• Degree in Social Work or a related professional field with KS/MO license
• Minimum of three years of experience working with older adults in healthcare, the community or care facility setting
• Highly self-motivated, organized, and capable of working independently
• Strong collaboration skills and ability to work effectively within a team
• Skilled in conflict resolution and a solution-oriented approach
