Position: LPN (Licensed Practical Nurse) Home Health

We are currently seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our home health team.

Responsibilities
  • Provide direct patient care under the supervision of a registered nurse.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor and record patients' vital signs and health status.
  • Assist with daily living activities, including bathing, dressing, and grooming.
  • Educate patients and their families about health management and home care.
  • Maintain accurate and timely patient records and documentation.
  • Communicate effectively with healthcare team members to ensure optimal patient care.
  • Current LPN licensure in the state of practice.
  • Valid CPR and BLS certification.
  • Previous experience in home health care or a similar setting is preferred.
  • Strong assessment and critical thinking skills.
  • Ability to work independently and manage time effectively.
  • Excellent communication and interpersonal skills.
  • A genuine compassion for patients and a commitment to quality care.
