Job Summary

As an RN Case Manager of Luminos Hospice you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best in class hospice care.

Responsibilities
  • Coordinates and oversees all aspects of patients' home care.
  • Collaborates with patient care teams to develop and implement care plans.
  • Educates families on patient condition, needs and care.
  • Performs head to toe physical assessments.
  • Checks vital signs and manages medications.
  • Administers and maintains catheters.
  • Complete documentation timely and accurately.
  • Supervises HHAs.
  • Regularly communicates patient progress to the clinical manager, physician, and care team.
  • Other duties as assigned.
Requirements Licensure
  • Current State license as a Registered Nurse.
  • Current driver’s license.
  • Current CPR Certification.
Experience
  • Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full‑time hospice preferred.
Skills
  • Nursing skills as defined as generally accepted standards of practice.
  • Knowledge of state and federal regulations.
  • Ability to work independently and make sound decisions.
  • Strong attention to detail and organizational habits.
  • Excellent verbal and written communication skills.
  • Good interpersonal skills.
Transportation
  • Reliable transportation and valid auto liability insurance.
