Position Summary

The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert who provides ethical, evidence-based, safe and compassionate nursing care. The RN Care Coordinator focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs, performing clinic related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes.

The RN Care Coordinator provides support to their assigned physicians by communicating with the patient/family/caregiver and relaying information to the physician, providing support and nursing care services during the physician's clinic sessions and ensuring the patient/family/caregiver understands the plan of care. The RN Care Coordinator verifies all orders placed during the clinic session have been completed and the physician has been notified of the results.

Responsibilities

and Essential Job Functions
  • Manage patient flow, triage and direct patient care.
  • Liaise between front desk staff, nursing staff, physicians, and clinic manager.
  • Develop expected patient outcomes, define resource utilization and work with provider to set patient goals.
  • Provide nursing support/coordination to optimize patient outcomes and improve patients’ quality of life through clinical and educational programs and/or support services.
  • Support patient self‑management of disease.
  • Coordinate continuity of care with patients and families following hospital admission, discharge, and ER visits.
  • Manage high‑risk patient care, including those with multiple comorbidities or high risk for readmission, including a registry.
  • Develop relationships with patients and use assessment skills to identify physical, psychosocial, financial, and environmental health care needs.
  • Educate patients and ancillary personnel about health care needs, safety issues, and benefits, including patient education.
  • Utilize EMR to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
  • Observe, record and report patient’s condition and reaction to drugs and treatments to physicians.
  • Educate patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
  • Maintain and review patient records, charts and other pertinent information.
  • Record tests and examination results and follow up with patient.
  • Post tests and examination results.
  • Respond to and refer incoming patient treatment‑related phone calls.
  • Instruct patient and family regarding medications and treatment instructions.
  • Maintain patient confidentiality.
  • Document patient care using EMR to maintain or develop patient records in compliance with practice standards.
  • Perform professional, clinical and/or technical competencies of the assigned unit or department.

Required

Education and Experience
  • High School Graduate.
  • Bachelor's Degree in Nursing.
  • 2 or more years of RN experience.
Preferred

Education and Experience
  • Previous medical office experience.
Licensure and Certification
  • Licensed Registered Nurse (LRN) – Multi‑State or single‑state RN license in good standing in the state where you will be working.
  • Must hold a current active multi‑state RN license in good standing or an active single‑state RN license with proof of application of a multi‑state RN license in the state in which you live.
  • Staff hired prior to 2/1/2024 must obtain a multi‑state RN license from the state in which they live by 4/30/2026.
  • Basic Life Support Heartsaver (BLS HS) – American Heart Association (AHA) or obtained within 14 days.
Basic Knowledge, Skills, and Abilities
  • Basic typing and word‑processing ability.
  • Excellent analytical, critical thinking and problem‑solving skills.
