In-Home Nurse Practitioner; Per Diem - Salina, KS

Job Summary

Per Diem Nurse Practitioner – In‑Home Health Evaluations at Signify Health, part of CVS Health®.

• Conduct in‑home visits lasting about 45 minutes to evaluate patients, review medication and medical history, perform physical assessments, and order additional diagnostic tests as needed.
• Use iPad and clinical workflow tools; not required to prescribe medication or alter treatment regimens.
• Work with Signify Health as a clinician colleague to fulfill role requirements.
• Provide virtual visits when necessary.
• Offer diagnostic and preventive services such as spirometry, diabetic eye exams, DEE, etc.
• Travel locally and within licensed states; flexible schedules required.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Anticipated weekly hours: 12. Part‑time. Pay range: $37.66 – $81.11 per hour. Per diem employees qualify for hourly base rate plus performance incentives; malpractice insurance coverage during Signify Health activities; supplies and other perks.
