Ascend Hospice is seeking a dedicated and compassionate Hospice Registered Nurse Case Manager (RN) to join our team in Overland Park, KS. As a valued member of our team, you will play a crucial role in providing high‑quality care to patients and their families, while also supporting the overall mission of our organization.

As a Hospice Registered Nurse Case Manager (RN), you will be responsible for assisting the team with on‑call, case management, and visits to aid in coverage and call outs. Your duties will include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re‑evaluating patients’ nursing needs
• Using assessment data to determine care plan needs
• Collaborating with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well‑coordinated
• Counselling the patient and family in meeting end‑of‑life needs
• Initiating appropriate preventative and palliative nursing procedures
• Communicating with the physician regarding patient’s needs and reporting any changes in the patient’s condition
• Supervising the Hospice Aides

• Associate's Degree in Nursing
• State Certified RN license
• 2 years’ experience as a professional nurse, hospice experience preferred
• Valid driver’s license and reliable transportation
• Willingness to travel
• Strong verbal and written communication skills
