MDS Coordinator (RN)

We are seeking a highly skilled and compassionate MDS Coordinator (RN) to join our team at Overland Park Respiratory & Rehab Center in Overland Park, Kansas. As a valued member of our care community, you will play a vital role in delivering exceptional patient care and contributing to our mission. Job Summary

As a MDS Coordinator (RN), you will be responsible for conducting initial and comprehensive assessments of our residents, developing and implementing individualized care plans, and leading and participating in care plan conferences. You will also collaborate with our interdisciplinary team to ensure high-quality, resident-centered care. This is an in-person role.

Responsibilities
  • Conduct initial and comprehensive resident assessments using the Minimum Data Set (MDS) and develop individualized care plans
  • Lead and participate in care plan conferences to ensure comprehensive care
  • Collaborate with the interdisciplinary team to develop and implement resident care plans
  • Identify and prioritize resident needs, and coordinate care to meet those needs
  • Maintain accurate and timely records of resident assessments, care plans, and progress
  • Participate in quality improvement initiatives to enhance resident outcomes and care
Requirements
  • Current licensure as a Registered Nurse (RN) in the State of Kansas
  • Experience as an MDS Nurse
  • Strong clinical skills and knowledge of gerontological nursing practices
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong attention to detail and ability to prioritize tasks effectively
