RN Case Manager – Telephonic​/Field – and North Kansas

Overview

RN Case Manager I – Telephonic / Field – Central and North Central Kansas. Field-based role requiring travel to client sites or designated locations and occasional office attendance for meetings or training. Must reside in the state of Kansas. This position is not eligible for employment-based sponsorship. Travel: 10–15% in Central/North Central Kansas to visit members in person. Counties supported:
Shawnee, Jackson, Wabaunsee, and Pottawatomie.

Work Hours:

8:00 a.m. – 5:00 p.m., Monday through Friday.

• Perform care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum.
• Conduct assessments virtually and in person (e.g., hospitals, community centers, homes) to identify individual needs and develop specific care management plans with objectives and goals.
• Implement care plans by facilitating authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements.
• Coordinate internal and external resources to meet identified needs; monitor and evaluate effectiveness of the care management plan and modify as necessary.
• Interface with Medical Directors and Physician Advisors on the development of care management treatment plans; negotiate rates of reimbursement as applicable.
• Assist in problem solving with providers, claims or service issues.

• Requires BA/BS in a health-related field and minimum of 3 years of clinical experience; or equivalent combination of education and experience.
• Current, unrestricted RN license in the state of Kansas.

• Computer proficiency preferred.

• Region/Scope:
  Supports Region 2; travel to visit members in Central/North Central Kansas; on-site visits may occur at hospitals, community centers, or member homes.

  Hybrid/virtual work considerations; mileage reimbursed for travel.
