Position Overview

Breaking Barriers KC is seeking a compassionate, organized, and detail-oriented Registered Nurse (RN) to oversee the medical coordination and health-related needs of individuals receiving services. The RN will collaborate with physicians, guardians, pharmacies, hospitals, and interdisciplinary team members to promote positive health outcomes while ensuring compliance with KDADS, HCBS, Medicaid, and company standards.

This position also plays a key role in Quality Assurance and Performance Improvement (QAPI), helping ensure services meet regulatory requirements and person-centered best practices.

Essential Responsibilities Clinical Care
  • Coordinate and monitor medical care for individuals receiving services.
  • Complete nursing assessments and individualized health care plans.
  • Review physician orders and ensure timely implementation.
  • Monitor chronic health conditions and communicate changes to the interdisciplinary team.
  • Review medication regimens for accuracy and safety.
  • Assist with medication-related concerns and provide nursing consultation.
  • Support staff with health-related training and clinical guidance.
Quality Assurance & Compliance
  • Conduct Quality Assurance reviews of Medication Administration Records (MARs).
  • Audit nursing documentation, physician orders, consents, and health records.
  • Maintain compliance with KDADS, HCBS Final Rule, Medicaid, and company policies.
  • Assist with investigations, incident reviews, corrective action plans, and quality improvement initiatives.
  • Participate in internal audits and licensing reviews.
  • Monitor clinical documentation for completeness and regulatory compliance.
Care Coordination
  • Collaborate with physicians, pharmacies, hospitals, specialists, therapists, and healthcare providers.
  • Schedule medical appointments and coordinate follow-up care.
  • Track annual physicals, laboratory testing, immunizations, and required health documentation.
  • Follow up after emergency room visits and hospitalizations.
  • Participate in Person-Centered Planning (PCSP), MFEIS, and interdisciplinary meetings.
  • Assist with appointments as needed.
Staff Education & Support
  • Provide clinical education and medication training for Direct Support Professionals.
  • Support ongoing staff competency development.
  • Assist with policy implementation and nursing best practices.
  • Promote infection prevention, safety, and person-centered care throughout the organization.
Qualifications Required
  • Current, unrestricted Kansas Registered Nurse (RN) license.
  • Associate Degree in Nursing (ADN) required;
    Bachelor of Science in Nursing (BSN) preferred.
  • Strong clinical assessment and documentation skills.
  • Excellent communication, organizational, and time management abilities.
  • Valid driver's license and reliable transportation.
Preferred
  • Experience working with individuals with Intellectual and Developmental Disabilities (I/DD).
  • Missouri RN licensure
  • Experience in behavioral health, home health, residential services, or case management.
  • Knowledge of KDADS regulations, HCBS Final Rule, Medicaid, and quality assurance practices.
  • Experience with electronic health records and medication administration systems
Compensation

Salary: $70,000–$90,000 annually, including performance-based bonus opportunities.
