RN Case Manager – Telephonic​/Field – South and South West Kansas

About This Role

The RN Case Manager – Telephonic / Field role involves managing care for members with complex and chronic needs in South Central and South West Kansas. This position is ideal for experienced nurses who thrive in a field-based environment and are committed to optimizing member health care.

• Location: Overland Park, KS
• Schedule: Full-time, Monday through Friday, 8am – 5pm

• Perform care management within the scope of licensure for members with complex and chronic care needs.
• Conduct assessments virtually and in person to identify individual needs.
• Develop and implement care management plans to address objectives and goals.
• Facilitate authorizations/referrals as appropriate within benefits structure.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate the effectiveness of care management plans.
• Interface with Medical Directors and Physician Advisors on treatment plans.
• Negotiate rates of reimbursement as applicable.
• Assist in problem solving with providers, claims, or service issues.
• Travel 10-15% within South Central to South West Kansas to visit members.

• Education: BA/BS in a health-related field required
• Experience: Minimum of 3 years of clinical experience required
