Home Health Physical Therapist

Mercy is seeking a compassionate Home Health Physical Therapist to provide high‑quality, in‑home rehabilitative care to adult and geriatric patients. In this role, you will perform comprehensive assessments, develop individualized treatment plans, and deliver evidence‑based interventions to improve mobility, safety, and quality of life. You’ll collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care using EMR/OASIS standards.

• Perform comprehensive in‑home PT evaluations for diverse adult and geriatric patients.
• Develop and implement individualized, evidence‑based treatment plans and goals.
• Provide gait, balance, strength, and functional mobility training to improve safety and independence.
• Educate patients and caregivers on exercises, equipment use, and fall prevention strategies.
• Document visits accurately in EMR/OASIS and ensure regulatory and agency compliance.
• Collaborate with nurses, physicians, and other disciplines to coordinate holistic care.
• Monitor progress, reassess needs, and adjust care plans accordingly.
• Contribute to Mercy’s culture of compassion, diversity, and continuous improvement.
• Participate in staff meetings, in‑services, and ongoing professional development.

• Active state Physical Therapist (PT) license.
• Doctor of Physical Therapy (DPT) or equivalent accredited PT degree.
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.
• OASIS documentation experience preferred.
