Sequoyah Pointe Living Center is seeking an experienced and compassionate Social Services Director to join our leadership team! We’re looking for a motivated professional who enjoys working with residents and families and wants to make a meaningful difference and grow their career in long-term care!
What You’ll Do:- Coordinate resident admissions and discharge planning.
- Complete required assessments, documentation, and resident interviews.
- Participate in interdisciplinary care plan meetings.
- Assist residents and families with needs, concerns, and available resources.
- Maintain accurate and timely documentation in Point Click Care.
- Advocate for residents and help promote their psychosocial well-being and quality of life.

- Long-term care experience required.
- Social Services experience preferred.
- Strong communication, organization, and problem-solving skills.

- PCC experience is a plus.
