Regional Physical Therapist (PT) – Southern & Southwest Minnesota | $15,000 Retention Bonus

Territory: Southern & Southwest Minnesota

Compensation: $110,000–$210,000 annually + $15,000 Retention Bonus

Schedule: Full-Time | Monday–Friday

What You’ll Do

• Evaluate patients throughout Southern and Southwest Minnesota in their homes.
• Develop individualized treatment plans that promote mobility, strength, balance, and independence.
• Recommend home safety modifications and adaptive equipment.
• Educate patients and caregivers to support long‑term success.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team.
• Complete timely, accurate documentation and monitor patient progress.

What We’re Looking For

• Active Minnesota Physical Therapist (PT) license.
• Excellent clinical judgment and communication skills.
• Passion for delivering high-quality care in community settings.
• Ability to work independently while managing a regional territory.
• Valid driver's license and reliable transportation.
