Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) +
Mileage Reimbursement

As part of our growing medical group, you'll provide on-site primary care throughout assisted living, memory care, and long-term care communities across the Twin Cities Metro. You'll play a central role in clinical decision-making, helping coordinate care, manage complex conditions, and guide treatment plans that directly affect patient outcomes.

Organizations such as Bluestone Physician Services, Genevive, and Lifespark have helped demonstrate the value of community-based medicine. Twin Cities Physicians continues to expand by placing experienced providers where they can have the greatest influence on patient care.

• Build expertise in geriatrics, chronic disease management, preventative medicine, and post-acute care.

• Provide on-site primary care and rounding services throughout assisted living, memory care, and long-term care communities.
• Perform comprehensive assessments, chronic disease management, and preventative care.
• Develop and adjust individualized treatment plans.
• Collaborate with nurses, therapists, caregivers, physicians, and facility leadership.
• Support transitions of care and help reduce avoidable emergency room visits and hospitalizations.
• Serve as a key clinical partner for patients, families, and community staff.

• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in geriatrics, family medicine, internal medicine, post-acute care, hospital medicine, or related settings preferred.
• Strong communication skills and confidence working independently.
• Valid driver's license and reliable transportation.
