Job Title: RN Case Manager – Lead with Compassion in Hospice Care
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Location: Greater Twin Cities Metro Area, 21-County Service Area (territories assigned based on coverage needs)

- Provide care in homes, assisted living, and skilled nursing environments
. - Maintain accurate, timely documentation to support quality outcomes.
- Active Minnesota Registered Nurse license.
- Experience in hospice, oncology, or chronic illness management
. - Strong communication, organization, and problem-solving skills.
- Ability to work independently while staying connected to the team.
