As a Floater Behavioral Health Nurse, you will be responsible for providing comprehensive behavioral health care services to patients as assigned. You will work collaboratively with a multidisciplinary team to support the mental health and well-being of individuals seeking care. This is a dynamic role that requires flexibility and adaptability, as you will be assigned to different areas within the clinic as needed.
Essential Functions:
- Assessment and Evaluation:
Conduct thorough assessments and evaluations of patients' mental health status, including psychosocial, emotional, and behavioral factors. Use evidence-based assessment tools to gather information and develop an understanding of patients' needs.
- Treatment Planning:
Collaborate with the patient, their family, and the healthcare team to develop comprehensive treatment plans based on the assessment findings. Incorporate patient goals, preferences, and evidence-based interventions into the plan.
- Direct Patient Care:
Provide direct care to patients, including individual counseling, group therapy, and crisis intervention. Deliver evidence-based interventions tailored to meet the unique needs of each patient. Monitor patients' progress and make necessary adjustments to treatment plans.
- Medication Management:
Collaborate with psychiatrists or other prescribing providers in the clinic to assist in medication management. Assess medication effectiveness, side effects, and adherence. Provide education to patients about their medications.
- Patient and Program
Education:
Educate patients, their families, and program staff about mental health conditions, treatment options, and self-care strategies. Empower patients to take an active role in managing their behavioral health by providing resources and teaching coping skills.
- Collaboration and Consultation:
Work closely with the interdisciplinary team, including physicians, psychologists, social workers, and other healthcare professionals, to provide integrated care. Consult with team members to discuss complex cases, treatment approaches, and ongoing patient care.
- Documentation and Reporting:
Maintain accurate and up-to-date patient records, including assessments, treatment plans, progress notes, and any other necessary documentation. Adhere to clinic policies and regulatory requirements related to record keeping. Complete infection control forms and incident report as needed.
- Community Outreach:
Participate in community outreach programs and initiatives to promote mental health awareness, prevention, and early intervention. Collaborate with community organizations and agencies to coordinate care and provide referrals as needed. - Continuous Professional Development:
Stay current with advancements in behavioral health nursing and evidence-based practices. Attend relevant training sessions, conferences, and seminars to enhance your clinical knowledge and skills. - Other duties as assigned.
- Associate's degree in Nursing.
- Current and valid nursing license.
- Experience working in behavioral health, SUD settings preferred.
- Strong knowledge of mental health disorders, treatment modalities, and psychotropic medications.
- Excellent assessment, diagnostic, and therapeutic skills.
- Ability to work effectively in a multidisciplinary team and communicate collaboratively with patients and colleagues.
- Strong organizational and time management skills.
- Compassionate and empathetic approach towards patients.
- Knowledge of community resources and referral processes.
- Familiarity with electronic health record systems and proficiency in documenting patient care.
