Setting: 1 Skilled Nursing Facility (Post-Acute / Long-Term Care)

We are seeking an experienced Nurse Practitioner (NP) to serve as an in-house SNF-ist
, providing comprehensive primary and internal medicine care to geriatric residents in Skilled Nursing and Assisted Living facilities.

In this role, the Nurse Practitioner functions as the facility’s day-to-day clinical leader, working collaboratively with interdisciplinary care teams, attending physicians, and medical directors to deliver high-quality, patient-centered care. This position is ideal for NPs who are passionate about post-acute and long-term care and want a predictable weekday schedule with meaningful clinical impact.

• Serve as the primary internal medicine provider within assigned skilled nursing facilities
• Manage day-to-day care for short-term rehabilitation and long-term care residents
• Perform comprehensive health assessments, physical exams, and differential diagnoses
• Diagnose and manage acute and chronic medical conditions under established protocols
• Prescribe medications and treatments as clinically appropriate
• Evaluate and manage residents experiencing changes in condition
• Lead and participate in Advanced Care Planning discussions
• Collaborate closely with nursing staff, interdisciplinary teams, attending physicians, and medical directors
• Coordinate referrals to specialists and external healthcare services
• Maintain accurate clinical documentation and demonstrate proficiency with CPT coding

• Facility-based nursing and interdisciplinary care teams
• Physician oversight and medical director support
• Established clinical protocols and workflows
• Administrative and operational support to streamline care delivery

• Active Advanced Practice Nurse (APN/NP) license in applicable state
• Active DEA registration
• Master’s Degree in Nursing (required)
• Minimum 1 year of experience in a Skilled Nursing Facility (preferred)
• Prior experience in nursing or related medical fields
• Strong leadership skills and team-oriented approach to care
• Excellent written and verbal communication skills
• Ability to build rapport with patients and their families
• Comfortable working in a fast-paced clinical environment
