We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team in Washington Township, OH. The ideal candidate will have experience in Memory Care and Medication Management, providing high-quality nursing care to our residents with cognitive impairments and other healthcare needs. Current Offering: $2000 Sign On Bonus.

• Provide direct nursing care to residents, focusing on Memory Care patients.
• Administer medications accurately and safely according to prescribed orders.
• Monitor and document residents’ health status and report any changes to the healthcare team.
• Assist in developing and implementing individualized care plans.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Educate residents and their families about health conditions and medication management.
• Maintain compliance with all regulatory and facility policies.

• Current and valid LPN license in the state of Ohio.
• Experience in Memory Care preferred.
• Strong knowledge of Medication Management and administration.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Compassionate and patient-centered approach to care.
