Licensed Practical Nurse; LPN

Home Health Licensed Practical Nurse (LPN) – Waverly, Ohio

• Provide skilled, in‑home nursing care to patients with chronic conditions, post‑surgical needs, or other health challenges.
• Take vitals, perform assessments, document care, and update the RN supervisor.
• Support patients in following their care plan and learning self‑care techniques.
• Educate family members on care routines, medications, and home safety.
• Work alongside a care team including RNs, CNAs, and physicians.
• Contribute to discharge planning and ongoing care coordination.

• Active Ohio LPN license.
• Graduate of an accredited LPN program.
• At least one year of LPN experience, ideally in home health.
• CPR certification.
• Familiarity with home health regulations (state and federal).
• Clinical confidence, strong communication, and a compassionate heart.

• Pay: $25.00 – $30.00/hour.
• 1:1 nurse‑to‑patient ratios providing focused, individualized care.
