Job Summary

Provides direct patient care under the supervision of a nurse and works collaboratively with the care team.

• Greets and orients new patients to the hospital environment, room, and unit routines.
• Creates a positive patient experience by focusing on what matters most to the patient and visitor(s).
• Fosters patient engagement by involving the patient and/or significant others in hand‑off to proactively meet patient needs and assure a safe environment through intentional rounding.
• Provides ongoing communication through regular updates to the patient‑room care board.
• Promotes a Culture of Safety by mindfully integrating Error Prevention tools into daily routines.
• Contributes to the assessment of patients by collecting data for review by a nurse and reporting abnormal findings.
• Documents patient progress toward goals and ensures documentation is accurate, complete, specific, and timely.
• Assists nursing staff to identify physical, psychosocial, age‑specific, and cultural considerations of each patient.
• Works with the nursing staff to provide personal care according to organizational standards and policies.
• Monitors tube systems and handles medication in accordance with medication safe handling practice; updates diet orders and verifies tray deliveries meet diet requirements.
• Electronically transfers patients in and out of the unit, facilitates direct admit processes, completes discharges in the electronic medical record (EMR), assembles paper‑lite charts, and assists with patient bed placement.
• Arranges for patient transport services and enters work orders to obtain supplies from material rooms.
• Entries staff assignments into the Responder 5 System and verifies staff have logged in and out.
• Performs continuous observation of EKG monitors for assigned groups of patients, communicates changes in cardiac rhythm to appropriate staff, interprets and documents rhythm, heart rate, and other measurements in the EMR.
• Runs telemetry strips when a significant change in rhythm, rate, or conduction occurs, scans all appropriate strips into the EMR per policy.
• Determines, reviews, and revises alarm settings for each patient based on physician orders and/or telemetry protocols and policies; troubleshoots alarm, monitor, battery, and equipment problems.

• Works under minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.

• No experience required.
• High school diploma, GED, or higher preferred.
• SRNA – State Registered Nurse Assistant (required upon hire).
• BLS – Basic Life Support (required within 90 days of employment).
• Adheres to all organizational policies and procedures.
• Behaves in a manner consistent with a culture of compliance, safety, and high reliability.
