Nurse Practitioner - Geriatric Care

SCHEDULE:

  • Full-Time
  • Monday to Friday

COMPENSATION:

  • $125,000 to $170,000 Annual Income
  • $105,000 to $125,000 Base Salary, dependent on experience
  • $20,000 to $45,000 Bonus Income (paid quarterly)

This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 50 physicians and nurse practitioners across Ohio and northern Kentucky, who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include primary care, psychiatric care, post-acute care, and transitional care.

POSITION

DESCRIPTION:

The nurse practitioner serves as the primary care provider for a dedicated panel of geriatric patients located within a defined geographic territory in the greater Dayton area. Patients are located in private homes (house calls), as well as long-term care facilities (ALFs, ILFs, SNFs) and other group settings.

Providers see a low number of patients per day, with schedules coordinated by a dedicated team factoring in type of visit, travel time, and patient needs. The schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.

Duties of the role include, but are not limited to:

  • health assessments to identify patient needs
  • develop personalized care plans
  • evaluate patient conditions and responses to treatment
  • adjust treatment plans as necessary
  • minor procedures and wound care as settings permit
  • educate patients and families on health management, disease prevention, etc.
  • work with physicians, nurses, and other professionals to coordinate care

The practice uses a mobile-based EHR with AI functionality to reduce administrative time.

On-call is phone triage only with low volume. Providers evenly share the on-call responsibility.

The practice offers a thorough training program and high level of support, welcoming new graduates interested in serving this vulnerable patient population.
