ESSENTIAL JOB DUTIES
  • Accepts referrals from care team regarding patients’ behavioral health needs. Responds in a timely manner and in a form of communication that is collaborative and reaches the care team effectively. Provides and documents referrals as needed to appropriate providers/resources both within and outside GLBHC.

  • Provides comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, and/or substance use needs, and/or social determinates of health needs. When assessment indicates, recommends referral to a different specialty provider, other member of the care team, and/or consultation. Provides counseling/behavioral health services to patients on an individual, couples, family, and/or group basis.

  • Assists the care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic conditions (mental and physical).

  • Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration. JOB SPECIFICATIONS
  • Education:

    Master’s degree in social work or counseling from an accredited graduate school. Basic Life Support (BLS) certification Required.
  • Licensure:
    Licensed as a Licensed or Limited Licensed Master’s Social Worker or Licensed Professional Counselor in the State of Michigan. Basic Life Support (BLS) certification Required. SUD:
