JOB SUMMARY:

Works cooperatively with the Director of Ancillary Services and dietary management staff to manage and/or direct all inpatient nutritional services, committees/subcommittees, department meetings, and other clinical responsibilities. Responsible for establishing and monitoring all continuous quality improvement issues for areas of responsibility per JACHO, state, and federal regulations. Provides nutritional instruction to patients and/or significant other and documents appropriately in the patient's computerized or non-computerized medical record.

Coordinates care with appropriate nutritional staff member(s) and/or medical staff physician(s) and is involved in a multi-disciplinary approach for the care of the patient. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.

PRIMARY

JOB RESPONSIBILITIES:

  • Conduct a comprehensive nutrition assessment for patients identified at nutritional risk, or as deemed necessary per policy, or by consult.
  • Incorporate evidence-based practice when implementing nutrition interventions.
  • Evaluate need for specialized nutrition support and make appropriate recommendations accordingly.
  • Appropriately document nutrition assessment in the electronic medical record.
  • Reassess the nutrition care process and implement changes as indicated.
  • Responsible for developing and directing the overall nutrition and hydration plan of care for each resident.
  • Recognize the varied needs of age-specific populations, as well as cultural, religious, and ethnic concerns.
  • Evaluate residents for significant weight changes, new admissions, new or continuing wounds, attend standards of care, care conferences, meal rounding, menu evaluation, and prepare LTC unit for survey readiness.
  • Participate in quality improvement projects as needed.

This is a full-time position for a registered dietitian to work in a long-term care and inpatient setting. Approximately 40% of time worked will be LTC, 50% of time worked will be inpatient, and 10% on project improvement/other tasks.
