Position Overview
As a Center Well Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.
Responsibilities- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self‑care techniques.
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes; participate in care coordination activities and discharge planning.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting.
- Nursing experience in a Home Health or Hospice setting.
Pay per visit/unit: $33.00 – $47.00
Annual salary range: $53,000–$73,000 per year (based on location, experience, and qualifications).
Additional InformationTuberculosis screening will be required for this patient‑facing role as part of Humana's program.
Driver safety program requires a valid state driver's license, personal vehicle liability insurance, and adherence to state minimum limits or specified liability limits.
