What You’ll Do

• Be the Clinical Lead:
  Perform comprehensive assessments and manage acute and chronic conditions in complex, medically fragile populations.
• Drive Innovation:
  Utilize telehealth and evidence‑based strategies to enhance patient outcomes and improve the overall care experience.
• Collaborate & Connect:
  Act as the vital liaison between hospitals, families, and facility leadership. You’ll be the "go‑to" expert for nursing staff and therapy teams.

What You Bring to the Team

• The Credentials: A Master’s or Doctorate from an accredited program (NP or PA) with a current state license, Board Certification, and an active DEA.
