A thriving, patient-centered primary care practice in Southwestern Connecticut has an opening for an experienced Nurse Practitioner. This well-established clinic is known for its highly experienced, collaborative care team and strong patient relationships.


Position Details

Setting: 100% Outpatient – Adult Medicine (18+)

Schedule: Monday, Tuesday & Friday

Call: No call required


What You’ll Do

  • Provide comprehensive primary care services to patients ages 18+
  • Conduct routine physical exams and health assessments
  • Diagnose and manage acute and chronic conditions
  • Develop individualized treatment, education, and care plans
  • Collaborate closely with physicians and interdisciplinary care teams
  • Coordinate referrals to specialists as needed
  • Deliver high-quality, patient-centered care in a supportive environment

Qualifications

Education: APRN – Adult or Family Nurse Practitioner

Licensure: Eligible for or active Connecticut APRN license

Experience: Minimum 1 year in a primary care or outpatient setting preferred
