Responsibilities

• Provide comprehensive psychiatric evaluations and ongoing medication management for post-acute and long-term care residents
• Develop, implement, and adjust individualized treatment plans in collaboration with interdisciplinary teams
• Monitor patient progress, document clinical findings, and ensure regulatory and documentation compliance
• Educate residents, families, and staff on behavioral health conditions, treatment options, and coping strategies
• Coordinate care and communicate with physicians, nursing staff, and external providers to support continuity of care
• Participate in quality improvement initiatives and contribute to a culture of compassionate, resident-centered care
