Court Liaison

Communi Care is seeking a Court Liaison to coordinate communication between our facilities, residents, families, legal representatives, and the court system. In this role, you will manage court-related documentation, schedule and track hearings, and ensure residents' legal and care needs are clearly represented. You'll collaborate closely with clinical and social services teams, provide updates to stakeholders, and help resolve issues impacting placement, treatment, or discharge.

This position requires strong organization, professionalism, and compassion in supporting resident-centered, legally compliant care.

Responsibilities

• Serve as primary liaison between Communi
• Care facilities, residents, families, attorneys, and court personnel
• Coordinate, track, and document all court hearings, orders, and legal communications related to residents
• Prepare, organize, and maintain accurate court-related and clinical documentation
• Communicate court outcomes and requirements to interdisciplinary clinical and social services teams
• Advocate for resident needs, ensuring care plans reflect legal directives and court decisions
• Support discharge planning and placement decisions impacted by court involvement
• Maintain strict confidentiality and compliance with HIPAA and organizational policies
• Identify and escalate complex legal or risk issues to leadership and appropriate stakeholders
• Participate in team meetings and care conferences to align legal, clinical, and social needs
