Gentiva Registered Nurse (RN) Hospice – Oxford, North Carolina

Gentiva is seeking a Registered Nurse (RN) for hospice care, offering a sign‑on bonus and comprehensive benefits.

Provide personalized, meaningful hospice and palliative care to patients facing life‑limiting illnesses, supporting patients and families with dignity, comfort, and love.

• Primary point of contact for patients and families, guiding them through hospice journey.
• Deliver and document skilled, hands‑on nursing care based on individualized Plans of Care in home or assisted living settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with interdisciplinary team—physicians, LPNs, CNAs, social workers, chaplains—to build and refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones, offering education, comfort, and bereavement guidance.
• Teach caregivers how to care for their loved ones safely and confidently.
• Communicate important updates in patient condition to the care team for timely interventions.
• Maintain accurate, up‑to‑date clinical documentation and follow infection control and compliance standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
• Help patients and families navigate medication administration, hospice philosophy, symptom and pain management, and end‑of‑life processes.

• Perform comprehensive patient assessments and reassessments, including in‑person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and determine visit frequency.
• Coordinate care across interdisciplinary teams, including physicians and nurse practitioners.
• Provide patient and caregiver education regarding disease process, self‑care techniques, end‑of‑life care, medications, nutrition, and dietary needs.
• Communicate changes in condition or signs of decline to appropriate provider.
• Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals.
• Document timely clinical records on all patients.
• Maintain effective working relationships with the interdisciplinary group and involve the team when needed.
• Participate in on‑call and patient support activities, conducting on‑call services in a competent and responsive manner.

• Education: Registered Nurse degree.
• Experience: Minimum one year of RN experience in acute care hospital, oncology, long‑term care, hospice or home health. Hospice or palliative care experience preferred.
• Licenses, Certifications, and Registration: Current RN license in the state of employment, CPR certification, valid driver’s license, automobile insurance.
